The following seven steps offer a framework for implementing patient decision support in primary care.

Assess patients' and practitioners' information and decision making needs. -

Review patient decision support tools to find those relevant to the identified needs. -
Step 5: Education and Training
Provide opportunities for practitioners to enhance their decision support skills. -
Step 6: Implementation
Implement decision support using strategies customized to the clinical setting. -
Step 7: Quality Monitoring Tools
Assess and document the quality of decision support and patient decision making.
