If you have been prescribed Coumadin or Warfarin and your family doctor (primary care provider) is part of DHMC's Internal Medicine Department, you will be referred to our Anticoagulation Clinic.

We treat patients with the following conditions:

• Atrial Fibrillation
• Deep Venous Thrombosis
• Heart Valve Replacement
• Hypercoagulable States
• Pulmonary Embolism

The success of your anticoagulation therapy depends absolutely on your involvement. To participate in our program, you must take your medication as prescribed and attend all scheduled visits.
