Step Three: Mail in Registration Fee
Send in the Bariatric Surgery Program fee in order to become registered.
Step Four: Register Online
Register for the Bariatric Surgery Program at hitchcock.remedymd.com.
Step Five: Program Packet and Checklist
Gather needed information for your program packet, including a psychological examination.
Step Six: Bariatric Educational Seminars
View online three bariatric educational seminars and for each seminar, take an online quiz for credit.
Step Seven: Weight Loss
Ongoing weight loss is highly encouraged before surgery to show your commitment to making nutritional and lifestyle changes.
Step Eight: Evaluations
Attend evaluation meetings with a nurse practitioner, dietitian, representative from Patient Financial Services, and the bariatric surgeon.
Step Nine: Final Instructions
Attend a pre-operative class and receive other instructions once your surgery date is set.
