Clinical Study Budget Documents and Routing Form

Here you will find documents that will assist you in developing a budget for your clinical research projects.

A Billing Plan (XLS) that establishes standard of care and delineates responsibility for payment for study costs is required for each clinical research proposal. This will help you in developing an internal budget that recognizes all costs associated with the study and assist PIs, study coordinators and coders in properly billing study charges.

The Budget Template will allow you to record all costs associated with your study. A list of resources is provided at the bottom to assist you in establishing study costs. If you don't find what you need, please feel free to call the Hitchcock Foundation office so we can provide assistance or point you to experts to answer your questions.

All studies must have a Routing Form signed by your department chair before a contract can be signed for sponsored research. Note:This form is in Excel format and consists of two pages. Please use the tabs at the bottom of the page to access both pages.
