Care Management for Employees

Continuing care managers are available to assist employees, their families, and retirees who have complex medical conditions or chronic illness, or who need assistance in navigating the healthcare system.

Our care managers can:

  • Advocate and negotiate for needed health and social services
  • Provide education about opportunities to maintain or improve health
  • Provide information and referrals to community resources
  • Facilitate authorizations as required and assist with explanations of medical insurance benefits (EOBs)
