About OCM

Our history

The Office of Care Management was formed in September 1996. The hard work of many people became a reality as we merged roles and took on more responsibilities. We were known briefly as Case Management but the staff officially decided to change the name to Care Management, as it reflected more the patient focus of our department. 

In November 1996, the department went away for a full day retreat to work on our Mission and Vision and the start of a strategic plan. Teams were designed and our primary focus for our first year was education/orientation. 

For the first few years of our development, our team memberssuch as social workers, continuing care managers, clinical resource coordinators and resource specialistshelped create their new roles and responsibilities. Other critical programs in our department include Lifeline, Patient & Family Relations, and the Managed Care Office. We also have a supporting cast of staff including our systems coordinator, data analyst, appeals coordinator, office staff, and the management team.
