It is extremely important that we receive a copy of any medical and developmental records related to the reason for referral, such as a summary of concerns from your primary care provider, growth curves, developmental evaluations, laboratory testing, and other specialty medical consultations. This information should be sent to us prior to your appointment so that we can better address your needs. If you have questions prior to scheduling an appointment, please call and ask to speak with one of our genetic counselors.

Please arrive 10-15 minutes prior to your scheduled appointment time in order to register.
